Please note that this form should ONLY be used for clients who need help resetting or unlocking their  HESC PIN account assistance or for verifying user identity.   If you need assistance with any other issue, please schedule an appointment to speak with a HESC representative here.  Please note inquiries not related to HESC PIN or user identity issues, will not be responded to.
